Overview

Morton’s neuroma, also called Morton’s metatarsalgia, Morton’s disease, Morton’s neuralgia, Morton metatarsalgia, Morton nerve entrapment, plantar neuroma, or intermetatarsal neuroma is a benign (non-cancerous) growth of nerve tissue (neuroma) that develops in the foot, usually between the third and fourth toes (an intermetatarsal plantar nerve, most commonly of the third and fourth intermetatarsal spaces). It is a common, painful condition.

Causes

The exact cause is unknown. Doctors believe the following may play a role in the development of this condition. Wearing tight shoes and high heels. Abnormal positioning of toes. Flat feet. Forefoot problems, including bunions and hammer toes. High foot arches. Morton neuroma is more common in women than in men.

Symptoms

Neuroma pain is classically described as a burning pain in the forefoot. It can also be felt as an aching or shooting pain in the forefoot. Patients with this problem frequently say they feel like they want to take off their shoes and rub their foot. This pain may occur in the middle of a run or at the end of a long run. If your shoes are quite tight or the neuroma is very large, the pain may be present even when walking. Occasionally a sensation of numbness is felt in addition to the pain or even before the pain appears.

Diagnosis

Your health care provider can usually diagnose this problem by examining your foot. A foot x-ray may be done to rule out bone problems. MRI or high-resolution ultrasound can successfully diagnose Morton’s neuroma. Nerve testing (electromyography) cannot diagnose Morton’s neuroma, but may be used to rule out conditions that cause similar symptoms. Blood tests may be done to check for inflammation-related conditions, including certain forms of arthritis.

Non Surgical Treatment

Ice therapy and anti-inflammatory medications or supplements. If conservative care measures fail to resolve your problem, some foot care providers may recommend a cortisone injection around your involved nerve to help reduce your swelling and inflammation. Concentrated alcohol injections around your affected nerve have also shown good results and should be considered before undergoing neurectomy, a surgical procedure to remove the enlarged, traumatized portion of your involved nerve.Morton neuroma

Surgical Treatment

Operative treatment of Morton?s neuroma should be entertained only after failure of nonoperative management. Standard operative treatment involves identifying the nerve and cutting (resecting) it proximal to the point where it is irritate/injured. This is usually done through an incision on the top (dorsal) aspect of the foot, although in rare instances, an incision on the sole (plantar) aspect of the foot maybe used. An incision on the sole of the foot works very well, unless an excessive scar forms in which case it can be problematic. Some physicians will attempt to treat Morton?s neuroma by releasing the intermetatarsal ligament and freeing the nerve of local scar tissue. This may also be beneficial.
